National School Lunch Program (NSLP) Calculation Data for Schools and LibrariesBackgroundAmendments to the Telecommunications Act of 1996 make Schools and Libraries eligible for Universal Service Fund (USF) discounts ranging from 20 to 90 percent on telecommunications services, Internet access, and internal connections. The discount percentage for a school/library is determined by whether it is located in an urban or rural area, and the economic needs of its students. Urban/rural designation is defined by county and based on Metropolitan Statistical Area data evaluated by the SLD. The level of economic need is determined by the percentage of students eligible for the National School Lunch Program. Urban/Rural DesignationEach Michigan county is listed in the table reference below along with its respective "Urban" or "Rural" designation, as identified on the SLD web site at http://www.sl.universalservice.org/reference/msa/michigan.asp.
Form 471 Discount Calculation WorksheetsIn an effort to assist Michigan USF applicants and trainers obtain National School Lunch Program (NSLP) data necessary for the calculation of their USF discounts, the Office of the Michigan Information Network (MIN) prepares two worksheet documents each year. These calculation tables (worksheet documents) provide the NSLP data necessary for determining the USF discount for a school/school district or library/library consortia. This information may be helpful in completing Form 471, Block 4, Item 10c. The tables may also be of value to you in providing the required discount calculation documentation. The Form 471 and instructions may be found at http://www.sl.universalservice.org/reference/forms.asp. SchoolsA school/school district worksheet is available which provides NSLP data and the related USF discount percentage for each school listed on the worksheet. In addition, the weighted average discount calculation is listed for each school district. The weighted average discount calculation is based on the assumption that all schools in a school district are recipients of the shared service identified in Form 471, Item 21. Depending on each circumstance, it may be necessary to adjust this information to accurately reflect your situation as illustrated in the following examples:

In all cases, the applicant should carefully follow the official Form 471 instructions and program eligibility requirements. LibrariesA library’s discount level is determined by using the weighted discount rate of the school district in which it is located. Participation vs. EligibilityIt is important to note that the data contained in the Year 5 worksheets is based on enrollments and participation reported to the Michigan Department of Education in October 2000. You are allowed to use more recent data if it is available. Also, you may use eligibility data if it is available. Please keep in mind that the information provided in Block 4, Item 10c is subject to program audit and must be documented. The information provided in the following worksheets is arranged in alphabetical order by school district. The school district name and weighted average discount follows a listing of the schools within the district.
